1. Hypertension (High Blood Pressure)

Primary care physicians monitor blood pressure regularly, prescribe antihypertensive medications, and recommend lifestyle changes such as a low-sodium diet, regular exercise, and weight management to control blood pressure levels effectively.


2. Type 2 Diabetes
